Items you will need to bring to your Annual Recertification or First Appointment:
If any of your paperwork was lost or finished incorrectly please arrive 15 - 20 minutes early to complete new paperwork.
- Birth Certificate(s)
- Social Security Card(s)
- Drivers License
- Lease Agreement

For disabled applicants:
- A copy of your Social Security Disability award letter
- Notification from a physician indicated that you have met disability requirements
- Out of pocket medical expenses
- A copy of your most recent bank statement for checking, savings, pension, and any assets (IRA,Stocks,bonds,etc.)
- Check stubs from employment, as well as name and address of employer.
- VA disability pension or benefits information

For low income applicants:
- Check stubs from employment (at least three examples)
- Name and address of employer
- Out of pocket childcare expense information. This should be a letter from your childcare provider, with the name(s) of each child cared for, the cost per hour/week, and the cost per hour/week for the service.
- Child Support information; such as a letter from the Office of Child Support Enforcement indicated the amount you receive, for what child, and how often it is paid.
- Copies of your most recent bank statments for checking, savings, pensions, and any assest (IRA, stocks, bonds, etc.)
- Unemployment insurance information, Social Security award letter, Food Stamps information, TANF benefit information, which include the date the benefits started and the number of weeks you will receive them.
